  • 雄性激素类药物:庚酸睾酮 为雄性激素类药物,是睾酮丙酸酯与睾酮庚酸酯的复合制剂。作用与丙酮睾酮相同,能促进男性性器官及副性征的发育,对抗雌激素。还有蛋白同化作用,可使肌肉增长、体重增加等。作用快,且持续时间较长,注射250mg可维持药效2~4周。临床上适用于治疗男性性腺机能不全、性器官发育不良、不育症、无睾症和稳睾症等;也可用于女性功能性子宫出血、更年期综合征、乳腺癌和性器官癌;肝硬化、再生障碍性贫血、骨质疏松症等;消耗性疾病。 【不良反应】大剂量或长期使用,可引起水钠潴留和水肿现象。青年女性可出现男性化现象,如嗓音变粗、多毛、痤疮、阴蒂肥大等。前列腺癌、孕妇禁用。前列腺肥大及肝、肾功能障碍者慎用庚酸睾酮。治疗乳腺癌时,如发现血钙过高应立即停药。
  • 男性避孕药:庚酸睾酮即是一种激素酯,它能抑制脑垂体,促进性腺素释放,从而使睾丸间质细胞分泌的内源睾酮下降,最终达到阻断精子发生的目的。中科院动物所在对猴子进行试验时发现,每周注射1次庚酸睾酮,一周后猴子的精细胞开始凋亡,到第五周精细胞凋亡达到高峰。对308位志愿者的临床试验还表明,庚酸睾酮避孕药对亚洲男性的作用明显高于白种人。在这项研究中,虽然精子的减少率并未呈现种族间的差异,但亚洲人比白种人对激素导致的精子发生抑制更为敏感,95%以上的亚洲人可达到无精子,而只有40%至70%的白种人成为无精子。 庚酸睾酮是一种长效男性避孕药,可通过多种途径给药(每周2次至每3周1次,每次200~500mg,肌内注射) ,通过对100余例志愿者试用,结果表明,每周肌内注射250mg,10周后可使大多数男子产生无精子症或严重少精子症,停药一定时间后精子数均可恢复,不良反应同前。如将注射间隔延长至10~12d以上,用加倍剂量效果也不理想。药代动力学研究表明,本品注射后当即产生一个超生理水平的血高峰,此后血浓度迅速下降,说明每周给药的必要; 此项血高峰有可能直接刺激生精,故部分志愿者未能达到明显少精子症,少数志愿者精子密度下降后可再次上升。庚酸睾酮只需每周注射1次,似比丙酸睾酮优越; 但即使如此,也甚难为多数人所接受。其他两种睾酮制剂,即环戊丙酸睾酮及环己烷羧酸睾酮,亦需每周注射1次。
